Peak Zone Fitness Freeze Form - All membership freezes must be received at least 1 business day prior to the net payment due date in order to not be charged for the following month. No exceptions or refunds can be made. Note: Membership Freezes require all required information for processing. For a Non-Medical Freeze, there is a small monthly fee required as well as a 180 day limit on the freeze. Freezes will under no circumstance be retroactive. It is your responsibility to fill out this form BEFORE the date requested. For Medical Freezes, please attach below a copy of your proof of a medical disability in order for your freeze payment to be exempt.
